Homecare and Support (Home from Hospital) in North East Essex
Homecare and Support is a Home from Hospital and Admission Avoidance service delivered to those patients who are registered with a North East Essex GP.
Swan works in partnership with Swan, Anglian Community Enterprise and the North East Essex Clinical Commissioning Group. The aim of the service is to ensure that hospital discharges are carried out both rapidly and safely to avoid unnecessary longer hospital stays and to reduce the strain on the admission wards by supporting people at home for up to six days, helping them to live independently after an accident or health incident.
The service is available 7 days a week and operates between 7am and 10pm, with a rapid response delivery of 4 hours from original referral if received within working hours. If outside of working hours then the delivery deadline is 12 noon the next day. The service can help with:
- Home Safety Checks
- Fitting of immediate equipment to prevent trips or falls
- Domiciliary Care
- Befriending/emotional support
Referrals to the Homecare and Support service are made via a Health Care Professional and supported by a service request form.
The service is available in North East Essex only (Colchester and Tendring local authority areas with a GP registered with NEE CCG).

night owl service
Swan Care and Support provides a short six day Home from Hospital service, to enable
patients to be discharged from hospital with the reassurance that a carer will visit to assist them during the night.
The Night Owl Service operates between the hours of 11pm and 7am and is open seven days a week, 365 days a year for people in North East Essex. This service can complement the care provision of clients also using:
- Swan's Rapid Response Service
- Falls Prevention
- Adult Social Care
- and other care services.
Swan Care & Support Service provides professionally trained Care and Support Assistants who provide services for up to six days or twelve visits, whichever comes sooner.
Personalised care and risk management plans are carried out to ensure we deliver a service centered around the individual. In addition, we provide information, advice and signposting services.
The Care and Support Assistants work in pairs and are able to assist with a variety of tasks.
